Our practice acknowledges patients may request access to their medical records. We require you to put this request\u00a0 in writing, this can either be hand delivered or posted to the practice. We generally respond within 24 hrs depending\u00a0 on the nature and size of the information requested. Patient Health Summaries are free of charge and supplied on\u00a0 the day of request. Full patient medical records supplied on a CD attract a $10.00 fee if picked up from our reception. If posted, an additional $5.00 is payable to cover the costs, a total of $15.<\/p>\n

Our practice will take reasonable steps to correct your personal information where the information is not accurate or\u00a0 up to date. From time to time, we will ask you to verify that your personal information held by our practice is correct\u00a0 and current. Upon receiving the complaint the Practice Manager will telephone the complainant if any further information is\u00a0 required and to acknowledge receipt of the complaint. You should expect a resolution to your complaint within 30\u00a0 days of receipt of the complaint.<\/p>\n
